A blank review aid
Multiple Account Review Map
These five prompts help organize a review of official information. Mark any prompt you want to keep in view. A mark has no legal meaning and does not show that an account or designation has been verified.
Keep all details in your own records. Do not enter beneficiary names, account or contract numbers, Social Security numbers, dates of birth, addresses or allocation details. There are no personal information fields in this checklist.
0 of 5 prompts marked.
Marks remain only in this open page. Reloading or leaving the page clears them. Nothing is sent to Sparshanaspabeautysalon or saved by this tool. A printed copy is under your control.
Use the prompts without making assumptions
This is a list of questions, not a completion score. For example, marking the question about official confirmation may simply remind you that a question remains unresolved. An unchecked prompt does not establish that information is missing.
Separate the labels
Notice which account categories or contract labels appear in the official materials you already use. Do not assume a single screen or designation describes every record.
Keep questions neutral
If the relationship between two records is unclear, ask which instruction applies to each one and which official record controls. This site cannot determine that relationship for you.
Route the question
TIAA can address its official records; the plan administrator can address plan requirements. Legal interpretation belongs with an attorney, and tax interpretation with a tax professional.
Print a useful reference
“Print blank checklist” produces an unmarked copy without changing the boxes on screen. “Print current marks” copies the marks shown in this tab. Both copies contain only these general prompts, the independent publisher notice and the page address.
Continue with the official questions
Prepare questions about coverage, separate contract records, contingent levels and any additional plan requirements. Ask the official source to resolve account-specific uncertainty.
